Li-Ve Tas and people with intellectual disability create toolkit for our sector

The Healthy dying for people with disability (HD4PWD) program supports people with intellectual disability at the end of life. It supports people’s goals and delivers the best possible outcomes in life and death.
The program does this by:
- building better systems and practices
- developing great communication
- educating and connecting mainstream health services.
The HD4PWD toolkit is for disability service providers, health care providers, carers and families in Tasmania. It is available for download from the Li-Ve Tasmania website livetasmania.org/hd4pwd
Li-Ve Tasmania developed the toolkit with people living with intellectual disability and their families. The Department of Social Services Information Linkages and Capacity Building (ILC) program provided funding.
